    • Assessment of Needs: Once you connect with the SEP agency, they will conduct an initial assessment to understand your specific needs and eligibility. This assessment is crucial as it determines whether you qualify for the IHSS program and identifies the type of services you require.
    • Eligibility Determination: The SEP agency will review your health status, current living situation, and the level of care needed. This includes evaluating your ability to direct your own care or the need to appoint an Authorized Representative.
    • Selecting a Service Provider: You’ll be guided through the process of selecting an IHSS-certified home health or personal care agency. This agency will be responsible for carrying out your care plan through a care provider of your choosing. This includes family members, friends, neighbors, etc. The IHSS Agency will conduct their own assessment and create the care plan using the state issued care plan calculator. 
    • Ongoing Support and Reviews: After the services commence, the IHSS agency and your case manager will continue to provide support and periodic reviews.
